We are excited to announce opportunities for residential providers to sponsor individuals receiving services through Virginia's ID/DD Waiver Program. If you are a potential sponsor provider with a commitment to fostering independence and providing quality care for individuals with intellectual and developmental disabilities, this is a great opportunity to support meaningful community integration and personal growth for our clients.
in this service, up to two individuals reside in the home of a professional provider, becoming fully integrated into the family. The family caregivers deliver the majority of required services, and may employ other trained and certified workers to assist with care. These substitute caregivers are thoroughly trained and instructed to carry out all services according to the Individual Plan for Supports (PFS). This service is available to adults of all ages Our dedicated staff offers ongoing support to create a nurturing and encouraging environment where individuals can live meaningful lives. Nexus Community Services provides a community stabilization program for adults in central Virginia. When someone is going through a psychiatric crisis, they may struggle with self-care. If your loved one is unable to care for themselves, is isolated from their community, or is at risk of homelessness, hospitalization, or incarceration, our community stabilization service can offer support.
To be eligible for the program, the individual in crisis should meet the following criteria:
- Be eligible for Virginia Medicaid.
- Be at high risk for psychiatric hospitalization, homelessness due to mental illness, or require intervention from the mental health or criminal justice system due to inappropriate behavior.
- Exhibit social isolation.
- Struggle to perform daily activities, putting their health and safety at risk.
- Be unable to recognize when they are endangering themselves.
Our Community Stabilization program accepts various types of referrals, including self-referrals, community referrals, and referrals from physicians, family or friends, schools, therapists, psychiatric providers, or the Department of Social Services.
